GP handover (SBAR)

Generates an SBAR handover — Situation, Background, Assessment, Recommendation — addressed to the patient's GP, for a patient you are leaving at home or referring on.

Using it​

Open the GP handover tool from the clinical tools grid and generate. You get a structured message you can edit, then copy into whatever route your service uses to contact the practice.

What it reads​

The whole structured assessment, the same as the other generators: history, examination, observations and NEWS2, scores, frailty and your free text.

Frailty is worth completing before you run this one. A handover for a CFS 7 patient that does not mention baseline function is a handover the GP has to phone you about.

What to check​

  • The Recommendation. It is a draft of what you are asking the practice to do, and it is the line the GP will act on. It must say what you actually want.
  • Safety-netting. What you told the patient belongs in the handover; make sure the draft reflects what you actually said.
  • Anything identifying. The message is going to a named practice about a specific patient — you will be adding identity at the point you send it, through your own systems, not in MedJot.

Queueing​

Available for queueing if it fails for lack of signal. Unlike a pre-alert, a GP handover is rarely time-critical to the minute, so this is the tool where queueing is most straightforwardly useful.
