Quick start

This walks through one report end to end. It should take about five minutes to read and rather less to do.

1. Sign in and complete your profile

Go to medjot.app and create an account with an email address and password, or sign in if you already have one — including one you made in ACTrack, which works here too.

The first time you sign in you will be asked to complete a short profile: name, email, phone, the month and year of your birth, your country and county, your clinical role and the sector you work in. You are also asked three separate yes/no questions about analytics, product update emails and marketing. Answering is required; agreeing is not, and "no" to all three changes nothing about how MedJot works.

The profile takes a minute and is asked once. Full detail, including what is deliberately not asked for, is in Your account and profile.

2. Install it (optional, but do it)

MedJot runs fine in a browser tab, but installing it to your home screen gives you a full screen, a faster launch and reliable offline behaviour. See Installing MedJot — it takes two taps and installs nothing.

3. Pick a presentation type

At the top of the app, choose the mode that fits the call:

  • Adult Medical — patients 16 and over, medical or trauma.
  • Child Medical — under 16, adds TICLS and paediatric assessment.
  • Mental Health — adds mental state examination and capacity.
  • Cardiac Arrest — RCUK ALS structure with 4Hs/4Ts.

Switching mid-report never deletes anything. Findings that the new mode has no field for are hidden, not lost, and reappear if you switch back. See Presentation types.

4. Work down the form

The sections run roughly in the order you would work: Presenting Complaint, Scene & Arrival, Primary Survey, Consent, Examination, Observations, Social History, Clinical History, then Plan & Outcome.

Tap a finding once to mark it positive, again for negative, again to return it to not assessed. There is no way to get stuck on a value you tapped by accident.

Watch the right-hand panel as you go — that is the report, and it is already written.

5. Record a set of observations

Open Observations and add a set. Type the numbers you took and tap the rest; anything you did not measure, leave empty. It will not appear in the report and will not be assumed normal.

If you record all seven NEWS2 parameters, the score appears in the footer beside Save. If you do not, it tells you which parameter is missing rather than scoring an incomplete set. See Observations and NEWS2.

6. Use a tool if you need one

The floating button at the bottom-left of the content area opens the toolbox — notepad, stopwatch, countdown timer, calculator and overdose calculator. Several can be open at once, and the stopwatch and timer keep running with their windows closed.

Scores such as Wells, AMT-10 and PHQ-9, and the falls, hypo and stroke care bundles, sit in the clinical tools grid.

7. Check and finish the report

The right-hand Live Preview panel holds the finished text. From here you can:

  • Copy it to the clipboard, to paste into your trust's ePRF.
  • Export PDF for the clinical record.
  • Lock the text to edit it by hand — after which tapping findings no longer overwrites your edits.

8. Start the next one

New Report in the sidebar clears the form. You are asked to confirm, because it cannot be undone.

Everything is autosaved as you work, so a refresh, a dropped signal or a phone that reboots mid-shift does not lose the report.
