Scores and assessments
MedJot carries a set of scoring tools. Each is completed in a modal and writes its result into the report text, so the score and the reasoning behind it end up in the record together.
They are reached from the clinical tools grid, and several also appear inline in the section they belong to.
The scores
| Tool | What it is | Where |
|---|---|---|
| Wells score (PE) | Pulmonary embolism probability | Clinical tools grid |
| Wells score (DVT) | Deep vein thrombosis probability | Clinical tools grid |
| AMT-10 | Abbreviated Mental Test, cognitive screening | Clinical tools grid |
| MMSE | Mini-Mental State Examination | Clinical tools grid |
| PHQ-9 | Depression severity | Clinical tools grid; Mental Health presentation |
| Cranial nerves | Structured cranial nerve examination | Clinical tools grid |
| TICLS | Paediatric assessment triangle — Tone, Interactivity, Consolability, Look, Speech | Child Medical presentation |
| 4Hs / 4Ts | Reversible causes in cardiac arrest | Cardiac Arrest presentation |
Two more scores live in the sections they belong to rather than the grid:
- GCS — in the Primary Survey (Disability) and in each observation set, always scored by E/V/M component.
- NEWS2 — derived automatically from each observation set. See NEWS2.
The Clinical Frailty Scale has its own page.
How scoring works
Answer the items; the score updates as you go. When you save, the result and its components go into the report.
The consistent rule: an incomplete assessment does not produce a total. As with GCS and NEWS2, a score that quietly treated unanswered items as normal would be a fabrication, so MedJot shows you what is outstanding instead.
Scores and the AI tools
Completed scores form part of the structured findings sent to the AI tools, which is what lets a differential list take account of a Wells score you already calculated rather than reasoning without it.
A word on what a score is
These are screening and probability instruments. MedJot calculates and records them; it does not interpret them, does not recommend a disposition on the strength of one, and does not flag a score as concerning. That reading is yours.
